Introduction
Tomey Physio, Sports & Acupuncture Clinic, established since 1993 by Karen Tomey, provides private physiotherapy and acupuncture services. The Clinic is committed to protecting your personal information when you use our Clinic services, as per current Data Protection (GDPR) legislation. This Notice explains the Clinics’ use of any information you give us and how we keep it safe and confidential. Karen Tomey is the Clinics’ Data Controller.
What information does the Clinic collect and who from?
We receive personal information about you (name, address, email and medical information etc), when you attend for treatment, when you contact us by email, text or phone and if a third party (e.g. Insurance Company, GP or Consultant) refers you to us.
How does the Clinic use your personal information?
We use your information to provide the best possible service to you, to protect your vital interests in case of medical emergency, to deal with your requests and enquiries and to email receipts/invoices. You will be asked for your Consent when sending any medical reports/letters about you. The Clinic does not use your personal information for marketing purposes.
How long does the Clinic keep your personal information for and why?
To abide by medical regulations, we keep your treatment notes for 8 years, or 8 years past your 18th birthday. As well as paper copies, upon discharge your notes are stored digitally onto an encrypted, password protected hard drive, as a security back up. We keep your personal data to assist us if you require treatment in the future, if solicitors request copies of your medical notes some time after your treatment, and billing accounts are kept for 6 years for tax purposes. We do not store your personal data on our Clinic computer.
When would we use your information to contact you?
We only contact you in reply to your enquiries, to confirm/change appointments during your treatment and soon after your last session to check your progress or for billing purposes.
What happens if you are under 18 years of age?
We require a parent/guardian to consent to you providing your personal information to us.
What are your rights under current Data Protection Legislation?
You can access your information by requesting, in writing, a copy of your treatment notes. We may request identification for this. You can request, in writing, a transfer of your personal information eg. to another clinic. You can ask the Clinic to rectify any errors in your personal information eg. change of address. You can request restriction or deletion of our use of your information. You can object to our use of your personal information.
What can you do if you have concerns?
